I. Job Summary

Utilizing the Nursing Process of assessing, planning, implementing and evaluating, the Licensed Practical Nurse is responsible for the delivery of patient care for adult and geriatric clients, and promoting collaboration among the multi-disciplinary health care team. Maintains open communi­cations and is available to component organizations and practitioner sites for consultation.

II. Duties And Responsibilities

Duties: Employee will be required to be readily available to work during any weather related incidents or other natural disasters and or/and any emergency situations that may arise.

III. Education

Must be Licensed Practical Nurse in State served by Agency

IV. Qualifications And Experience

At least one year nursing experience in an acute hospital setting preferred. Community Health Nursing
preferred but not mandatory.

SPECIAL KNOWLEDGE, SKILLS AND ABILITIES:
Due to the job responsibilities, the LPN position requires proficient knowledge in the use of Home Health Software, NetLeaming, and Hospital Meditech MIG-Internet Gateway.

Professional Requirements:
LPN License in state served by Agency.
CPR Certification
