I. Job Summary

Under the general direction of the Director of Case Management and in collaboration with physicians and hospital staff, evaluates and implements treatment regime for designated psychiatric and substance abuse patient population, assists with reporting suspected abuse for all age groups, assists uninsured with eligibility process, assists case managers with hospital follow up post discharge to prevent hospital readmission.

II. Duties And Responsibilities

Position Responsibilities

  • Take initial history, prepares psychosocial evaluation and care plans, including management risk for all consulted patients.
  • Consults with physicians and other clinical staff as necessary.
  • Provides clinical care to patients suffering from acute or chronic mental health or substance abuse problems.
  • Responsible for interviewing and investigating applicants and recipients to determine eligibility for medical assistance programs. Specialists will assist patients with applications, interviews, appointments, hearings and all necessary steps to obtain certification for medical coverage.
  • Assists in the development of protocols for specific psychiatric/substance abuse patient population.
  • Is knowledgeable of and utilizes appropriate community resources.
  • Participates in community outreach as appropriate to describe mental health programs.
  • Attends all safety training programs and describes his/her responsibilities related to general safety, department/service safety, and specific job-related hazards.
  • Follows the hospital exposure control plans for all pathogens.
  • Demonstrates respect and regard for the dignity of all patients, families, visitors, and fellow employees to ensure a professional, responsible, and courteous environment.
  • Promotes effective working relations and works effectively as part of a department/unit team to facilitate the department’s/unit’s ability to meet its goals and objectives
  • Additional tasks as assigned by the Director of Case Management

III. Education

Prefer master’s degree from an institution accredited by the Council on Social Work Education

Minimal of 1 year of acute psychiatric and/or crisis evaluation experience preferred.

Licensed in state at the LCSW level, LICSW preferred

IV. Qualifications And Experience

Works collaboratively with interdisciplinary staff internal and external to the organization. Participates in quality improvement and evaluation processes related to the management of patient care. The Social Worker is on-site and available Monday thru Friday.
